To register your child with Cedar Grove Childcare Centre, we require a parent interview to discuss your care requirements and our policies. This meeting will include a tour of the facility, an overview of the program and an introduction to various staff members. 

REGISTRATION FORMS & FEE

A completed registration form accompanied by a $100 registration fee per child is necessary to guarantee your child’s spot in the Centre.  The first $50 is considered an administration fee and the last $50 will be credited to your last month’s fee depending on a 30 day notice of withdrawal.

Information on the registration form must be completed in full including:

• child's legal name
• child's birthdate
• child's photograph
• immunization records
• Personal Health Number
• medical information
• parent contact information
• emergency contact information
• parent signature

Your signature is required on your child’s registration form, confirming that you have read and agree to the policies and procedures stated in this handbook.
